This is a U.S. government program that reduces my internet bill.

I have read and understand the program's benefits and rules at https://www.fcc.gov/ACP and https://ACPBenefit.org/.

It is your responsibility to keep your contact information (e-mail address, phone number, etc,) up to date, so you can receive important updates about this program.

This program is limited to one per household. A household is defined as anyone living at the same address who share income and household expenses.

This program is non-transferable to any other individual or household.

I may choose to participate in the Affordable Connectivity Program (ACP) from any service provider.

I may transfer the Affordable Connectivity Program benefit to another provider at any time during this program.

I have the right to file a complaint with Commission at https://consumercomplaints.fcc.gov

I allow Myakka Communications to transmit my information to the National Lifeline Accountability Database to enroll into the Affordable Connectivity Program.

I understand that I have to tell my service provider within 30 days if I no longer qualify for the ACP benefit, including:

I, or the person in my household that qualifies, no longer qualify through a government program or income no longer qualifies.

Either I or someone in my household gets more than one ACP benefit.

I agree that all of the information I provide on this form may be collected, used, shared, and retained for the purposes of applying for and/or receiving the ACP benefit. I understand that if this information is not provided to the Program Administrator, I will not be able to get ACP benefits. If the laws of my state or Tribal government require it, I agree that the state or Tribal government may share information about my benefits for a qualifying program with the ACP Administrator. The information shared by the state or Tribal government will be used only to help find out if I can get an ACP benefit.

All the answers and agreements that I provided on this form are true and correct to the best of my knowledge.

I know that willingly giving false or fraudulent information to get ACP benefits is punishable by law and can result in fines, jail time, de-enrollment, or being barred from the program.

Payment for the month is due on the 7th of the month. Account will be considered past due if not paid in full by the 10th of the month. If the account is not brought current by the 10th of the month, the customer's Internet plan will be reduced to the BASIC plan until the account is paid in full.

After 90 consecutive days of non-payment, your account will be put on hold and your service will be disconnected.

It is Client’s responsibility to call Company 48 hours prior to digging anywhere around the cable. Client fully understands that the fiber cable buried underground in Client’s yard is expensive to repair and, as such, will be responsible for any cost to repair damage done to cable by Client.
